EU nationals have the right to live and work in Poland for as long as they wish. However, in order to confirm your Polish residence, you must obtain a Residency Registration Certificate (Zaświadczenie o Zarejestrowaniu Pobytu Obywateli Unii Europejskiej w Polsce).
Confirm Polish Residence
According to the British Embassy in Warsaw, EU nationals must complete and sumit application form (Wniosek) in Polish. This form can be obtained from the Voivod Office. Along with the application confirming Polish residence, EU nationals must also provide photographic evidence of identity and nationality, such as their EU passport. The original will be required, along with 4 photocopies. In addition to photographic evidence of identity and nationality, EU nationals must also submit documents that confirm the purpose of their stay in Poland.

Non-EU Spouses of EU Citizens
If you are married to a non-EU spouse, they have the right to apply for EU Family Member Card (Karta Czlonka Rodziny Obywatela Unii Europejskiej) to stay with you in Poland.
Validity of EU Citizens’ Residency Card
Once an EU Citizens’ Residency Card has been issued, it remains valid with no need to exchange the card for a new Residency Registration Certificate. If you have lived in Poland for 5 years or more, you are entitled to apply for a permanent residency permit.
